The forms available in this directory should be used in conjunction with the applicable policy, procedure or guideline. Please ensure when submitting a form that is is complete and accurate.  

This form is to be used in relation to academic and non academic matters and must be used in conjunction with the CPEE Student Complaint and Appeal Policy and CPEE Academic or Non Academic Appeals Procedure.

CPEE may award Specified Credit or Unspecified Credit. The awarding of credit means that a student who successfully applies for credit shall have a reduced number of Units to complete their CPEE program to meet the requirement for graduation. For example, a CPEE student who has been awarded 2 Units of Credit in the Masters of Pavement Technology will need to complete (pass) a further 6 Units to meet graduation requirements.

This form is to be used by students to make application for Human Ethics Exemption. Applications must be made prior to commencing any research.

The following form is to be used when appplying to enter a CPEE or joint CPEE award program or a CPEE single unit enrolment. 

This form is to be used by students to make application for Restricted Access to and/or Confidential Examination of a student’s Industry Research Report or Work Based Project Report. Applications must be made at least 2 weeks prior to submission of the Report for examination. Students must refer to the information proved below under “Notes”, when completing this form. The completed form should be emailed to the CPEE HEO

This form is to be used by CPEE students when submitting assessment items during their studies. 

Any changes to your details must be updated via the submission of the following form. 

This form is to be completed:

  • When joining a CPEE committee and or
  • If you perceive that a conflict of interest may occure due to involvement with CPEE. 

This form is to be returned to CPEE prior to undertaking any associated work or committments. 

This form is to be used by new applicant who have been made an offer of admission but wish to defer their initial enrolment for that program.

The majority of units require students to complete an end of semester examination as part of their formal assessment. Students enrolled into these units are automatically registered for the examination following the completion of the semester enrolment/re-enrolment form.

Students are to nominate an examination venue ID with each semester enrolment.

Application is a Five Step Process

This guide is to assist potential students in completion and submission of an applications to study with CPEE via the public application portal. Prior to beginning an application, all potential students will need to agree that they have read and understood the document ‘Terms and Conditions of Admission, Enrolment and Re-enrolment’ in relation to study with CPEE.

This request form is to be completed prior to the start date of each semester in the event that you wish to purchase a printed version of the unit/s study notes to complement the primary electronic version.

This form is to be used by CPEE and CPEE joint program who wish to apply for an intermission of studies.

This form is to be used by current CPEE and CPEE joint program students for the submission of an assignment after the due date.
