This form is used for enrolled students to make one of the following amendments to their enrolment:
- Add a Unit
- Withdraw from a Unit
- Apply for a Program Extension
- Withdraw form a Program
- Early Exit from the MPT with a Graduate Certificate
Please make sure that you are aware of the financial and/or academic dates and related penalties that may be imposed when varying an existing enrolment: semester-dates
