This form is used for enrolled students to make one of the following amendments to their enrolment:

• Add a Unit
• Withdraw from a Unit
• Apply for a Program Extension
• Withdraw form a Program
• Early Exit from the MPT with a Graduate Certificate

 Please make sure that you are aware of the financial and/or academic dates and related penalties that may be imposed when varying an existing enrolment: semester-dates
